Insurance and Coverage Information

Understanding Your Chiropractic Benefits at Mid-Valley Chiropractic Allied Health Sciences

Navigating insurance coverage can feel overwhelming. At Mid-Valley Chiropractic Allied Health Sciences, we’re committed to transparency, accuracy, and communication. Our goal is to help patients understand how their benefits apply to chiropractic and allied health care, while also clarifying what our clinic can — and cannot — guarantee regarding insurance reimbursement.

Contracted and Preferred Providers

When a provider is contracted or preferred with an insurance company, it means we have a signed agreement to deliver covered services at negotiated rates. As a contracted provider, we honor those rates and billing procedures established by the insurer. Patients are generally responsible for applicable copayments, coinsurance, or deductible amounts, as defined by their plan.

At Mid-Valley Chiropractic Allied Health Sciences, both Dr. Gabriel Wilson and Dr. Marisa Aptecker are preferred (in-network) providers with:

  • PacificSource Health Plans

  • Moda Health Plans

  • BlueCross BlueShield

Additionally, Dr. Marisa Aptecker is a preferred provider with Samaritan Health Plans.

Good-Faith Benefit Verification

As a courtesy, our billing department verifies insurance benefits before your first visit. We rely on secure online portals and provider resources hosted by your insurer. These sources are useful but not definitive — information obtained is not a guarantee of payment or coverage.

We make every reasonable effort to confirm copayments, deductibles, and visit limitations, but final determination always rests with your insurance company. Patients are ultimately responsible for any balance due after claims are processed.

Plans Not Accepted

We are not contracted with Medicaid or any Medicaid-based plans, including:

  • PacificSource Community Solutions

  • Samaritan IHN (InterCommunity Health Network)

In addition, we do not bill or submit claims for:

  • Out-of-Network HMO Plans (HMOs require care from in-network providers only)

  • Out-of-Network Medicare Advantage Plans (including MA plans not listing our clinic as a preferred provider)

  • TriCare Health Plans

  • Kaiser Permanente / Kaiser Health Plans

  • HealthNet Plans

  • Any plan that does not offer out-of-network chiropractic benefits

Patients insured under these plans may still be seen on a self-pay basis, with itemized receipts available for personal reimbursement submission.

Out-of-Network Insurance and Courtesy Billing

When your plan is out of network, it means we do not hold a direct contract with that insurer. We may submit claims as a courtesy for plans that permit it, but reimbursement depends entirely on your policy’s out-of-network provisions and remaining deductible.

Common plans for which we provide courtesy billing include:

  • Providence Health Plans

  • First Choice Health

  • WebTPA

  • Other PPO networks offering out-of-network chiropractic benefits

Patients with these plans may be subject to balance billing — meaning any amount not covered by the insurer remains the patient’s responsibility.

Patient Responsibility and Insurance Contracts

It’s important to understand that your insurance policy is a contract between you and your insurer. Our clinic holds a separate provider agreement with the same company, but there is no contract between Mid-Valley Chiropractic and you as the insured party.

We encourage every patient to familiarize themselves with their specific coverage, including visit limits, copayments, and any exclusions. Ultimately, you are responsible for all fees not covered by your insurance carrier, including those applied to deductibles or services deemed non-covered.

Tribal Health Plans

We are honored to welcome Tribal Health Plan members to our practice. These plans often serve as a secondary “payer of last resort” when a private insurance plan is active, and as primary coverage when no other plan exists.

Tribal Health Plans typically cover chiropractic and therapeutic massage benefits in full, subject to any applicable copay. We take great pride in providing care to members of Tribal communities with the respect, compassion, and clinical excellence they deserve.

Transparency, Integrity, and Partnership

At Mid-Valley Chiropractic Allied Health Sciences, we value transparency in every aspect of care — including billing. Our front-desk and billing staff are happy to answer your questions, help you understand your estimated costs, and provide itemized receipts for personal reimbursement if needed.

We believe patients deserve clarity, and that begins with open communication and mutual understanding between our clinic, our patients, and their insurers.
